Dr. Dany Hanna, DO is a Urologist and fellowship-trained Gender Surgeon who specializes in gender-affirming genital surgery, including Metoidioplasty. At the Hanna Gender Center, Dr. Hanna works exclusively with transgender and non-binary patients from the greater Texas region and across the United States.
Dr. Hanna served in the Army National Guard as an Infantryman and later as a tactical intelligence officer. He later served as the Battalion Surgeon for 1-181st Infantry in the Massachusetts Army National Guard. He currently serves as the Medical Advisor for the Transgender Military Advocacy Organization SPARTA.
After his urology residency, Dr. Hanna completed a yearlong fellowship in gender-affirming surgery, with a special focus on Metoidioplasty. He also traveled to Belgrade, Serbia to train with Dr. Miroslav Djordjevic, who popularized the Belgrade Metoidioplasty procedure.
How Is Metoidioplasty Performed?
During Metoidioplasty surgery, the testosterone-enlarged clitoris is transformed into a small penis by cutting the ligaments that connect it to the pubic bone. Typically, Metoidioplasty size ranges from 1.5 to 4 inches in length.
There are three primary methods of Metoidioplasty:
- Belgrade Metoidioplasty, also known as Full Metoidioplasty, uses a tissue graft from the inside of the cheek (buccal mucosa) to lengthen the urethra into the phallus. Belgrade Metoidioplasty can be done as a 1-stage procedure that includes Vaginectomy, Scrotoplasty and placement of Testicular Implants.
- Simple Release, or Simple Metoidioplasty, is a less invasive type of Metoidioplasty. Since urethral lengthening is not performed in this procedure, urination while standing is not possible. Vaginectomy is also not done, although partial closure can be achieved. Scrotoplasty and insertion of Testicular Implants can be performed during the initial surgery. A primary benefit of Simple Release is that it is generally complication-free and comparatively inexpensive.
- Ring Metoidioplasty is similar to Simple Release with the added benefit of Urethral Lengthening. The urinary tube is created using mucosa from the inner labia in combination with a vaginal flap. A benefit of this approach is that you only have one surgical site to heal, unlike Belgrade Metoidioplasty which results in two surgical sites due to harvesting of the buccal mucosa graft. Patients can choose between Vaginectomy or partial closure. Scrotoplasty with testicular implants is done as a separate procedure, at least 6 months after the initial surgery.
Dr. Hanna uses Coloplast Torosa testicular implants (saline), which are the only FDA-approved implants. He can use solid silicone testicular implants by request.
Dr. Hanna can also provide complimentary procedures to optimize your Metoidioplasty results, such as a Mons Resection (pubic lift) to bring the penis into a more forward position, and other aesthetic revisions. As a Urologist, Dr. Hanna also has expertise with urological repairs for fistula and stricture complications.
Dr. Hanna works with a gynecologist and can refer you for a Hysterectomy. Many patients choose to have Hysterectomy done at the same time as Metoidioplasty.
After your Metoidioplasty surgery, Dr. Hanna will see you for several post-operative appointments and follow-up with you for a full year.
Surgery Requirements
Dr. Hanna is a member of the World Professional Association of Transgender Health (WPATH) and follows WPATH guidelines for surgery. You will need to provide two referrals from qualified mental health professionals, as well as a letter from your hormone provider stating you have been on 12 months of continuous hormones. (For the best Metoidioplasty results, a minimum of two years of testosterone hormone replacement is recommended.) Patients must be 18+ years of age.
